New Audiology Patients
What to Expect
When you first arrive at Ohio Head & Neck you will be asked to sign in at the front desk. Once the audiologist is available, she will meet with you to discuss your health history and specific hearing/balance/tinnitus difficulties. She will likely look into your ears with an otoscope and conduct several tests to determine your hearing and balance status.
Once a diagnosis is made, the audiologist will then take the time to explain all of your treatment options and answer any question you may have. We offer many styles and price levels of hearing devices. Our audiologist will work with you to create a treatment plan that fits both your lifestyle and your budget.

First time audiology patients patients may print and complete the form below prior to their arrival in order to expedite their office visit. You may either fax in the completed form to 330-492-0840, or bring it with you for the scheduled appointment.
